Free Pest Control Service Agreement Template
A handshake agreement works until a customer cancels mid-year, disputes a charge, or claims you never told them about the chemicals you applied. A written service agreement protects both sides and takes 10 minutes to set up.
Why You Need One
Verbal agreements are unenforceable. If a customer calls their bank and disputes a $150 charge, you need a signed document showing they agreed to the service, the price, and the terms. Without it, the bank sides with the customer every time. A signed agreement also sets expectations up front — what is covered, what is not, and what happens when pests come back between visits.
What to Include
Every pest control service agreement needs these sections:
1. Service Description and Frequency
Spell out exactly what you are treating, how often, and what pests are covered. "Quarterly pest control" is not specific enough.
Good example: "General pest management for ants, roaches, spiders, and crickets. Service includes interior baseboard treatment and exterior perimeter treatment (3-foot band from foundation). Quarterly visits: January, April, July, October."
2. Coverage and Exclusions
List what is covered and what is not. This prevents the "I thought termites were included" call in August.
Common exclusions: termites, bed bugs, wildlife, wood-destroying organisms, mosquitoes, fleas (unless specified), attic/crawlspace treatment (if not part of standard service).
3. Pricing and Payment Terms
State the per-visit price, annual total, and when payment is due. Include your accepted payment methods and any late payment fees.
4. Service Guarantee and Callback Policy
This is your competitive advantage. Most PCOs offer a callback guarantee — if pests return between scheduled visits, you come back at no charge. Define the window (usually 30 days) and any conditions (customer must not introduce new pest sources, must follow sanitation recommendations).
5. Cancellation Terms
How much notice is required? Is there an early termination fee? What happens to prepaid services? Spell it out. 30-day written notice is standard.
6. Chemical Disclosure
Many states require written disclosure of chemicals applied. Even where it is not required, it is good practice. Include a statement that the customer will be notified of chemicals used at each visit and that Safety Data Sheets are available on request.
7. Liability and Indemnification
Keep it simple. You carry insurance (list your policy number). You are not liable for pre-existing damage, customer negligence, or conditions outside the treatment area. Your liability is limited to the annual contract value.
8. Signatures and Date
Both parties sign. Include the date, the customer's printed name, and the service address. Digital signatures are legally binding in all 50 states under the ESIGN Act.

SERVICES
Provider will perform general pest management at the

and utility areas
- Exterior: perimeter spray (3 ft band), entry point
treatment, eave sweep for webbing
EXCLUSIONS
This agreement does NOT cover: termites, bed bugs,
wildlife removal, mosquito treatment, wood-destroying
organism inspections, or any pest not listed above.
PRICING
$[XX] per quarterly visit
$[XX] annual total
Payment due upon completion of each visit.
Accepted: credit card, debit card, ACH bank transfer.
Late payments (30+ days) subject to $15 late fee.
GUARANTEE
If covered pests return between scheduled visits,
Provider will perform a callback treatment at no
additional charge within 30 days of the previous visit.
Customer must report pest activity within 30 days of
last service.
CANCELLATION
Either party may cancel with 30 days written notice.
If Customer cancels before the contract term ends, no
early termination fee applies. Prepaid services will
be refunded on a pro-rata basis.
CHEMICALS
Provider will apply EPA-registered pesticides in
accordance with label directions and all applicable
federal and state regulations. Customer will receive
written notice of products applied at each visit.
Safety Data Sheets available upon request.
LIABILITY
Provider maintains general liability insurance
(Policy #: [Number]). Provider liability under this
agreement shall not exceed the annual contract value.
Provider is not responsible for pre-existing pest
damage, customer-introduced pest sources, or
conditions outside the treated areas.
TERM
This agreement begins on __________ and continues
for 12 months, renewing automatically unless
cancelled per the terms above.
